Atherosclerosis (excessive deposition of fat and related substances along the wall of blood vessels) and Buerger's disease (occlusion of small blood vessels of the legs secondary to smoking) are the main culprits for the severe pains in the legs. Both cause insufficient blood supply to tissues in the periphery. Initially the pain might appear when patient is active, but later it starts appearing at rest also. The irony is both are preventable if corrective measures are taken.
