Some of the refractory trigger points, which are wide- spread, might respond to injection of Botulinum toxin type A. This toxin is derived from Clostridium botulinum. It’s a di-chain polypeptide, which reduces muscle tone when injected into spastic and hypertrophic muscle. Usually, 5 to 10 units are injected at each trigger point and maximum up to 300 units. Each trigger part injected with 1 mL solution containing 10 units of toxin. The drug should not be repeated before 6 months for fear of antibodies formation.
