The sacroiliac joint (SIJ) is generally accepted as a potential source of low back and or buttock pain with or without lower extremity pain. Anatomically and biomechanically the sacroiliac joint shares all its muscles with the hip joint, thus it is unable to function in isolation. Subsequently the SIJ is subject to unidirectional pelvis shear, repetitive and torsional forces which can contribute to SIJ pain. Using intra-articular injections provocatively in normal subjects and diagnostically in chronic pain populations, the SIJ has been demonstrated to be a source of pain. It is widely accepted that fluoroscopically guided intra-articular SIJ block with local anaesthetic is the gold standard test for making the diagnosis of SIJ syndrome.
